Pills in the movies

Real life does sometimes appear in movies. You only have to think of Jack Nicholson in "Something's Gotta Give" to see an older man relying on a little blue pill to ensure a great performance with a hot young woman. Or possibly "Lonesome Jim" with Casey Affleck being nursed back to emotional and physical health by Liv Tyler.

Perhaps the real problem is we all feel too embarrassed by the reality of sexual dysfunction, we'd rather not see it on the big screen. It's not entertaining. Sex is not a factor

It's been interesting to watch the emergence and then control of feminism. When women first started asking for things like the vote, men slowly gave in but then made sure the changes had little effect - no female candidates to vote for. This manipulation of reality came under real pressure as the 1960's gave way to the 1970's and a new militancy was born. It seemed as though male privilege might actually crack. Well, we need not have worried. It may have taken a few more years than we expected, but the genie is now back in the bottle. Despite laws protecting women from discrimination, they are rarely used. Men continue to dominate in almost every area of activity and, remarkably, the majority of women have stopped publicly complaining. Well, perhaps there are odd moments when some speak out. You only have to think of Larry Summers and his remarks about the failure of women to excel in math, to see the possibility of change. But that possibility is swept away when you look at the recent midterm elections where more vitriol against candidates in general, and women candidates in particular, has been sprayed around without anyone thinking this odd. Think of witches and Halloween costumes to get the point. It seems everything is fair during elections.

When it comes to medicine, however, there are real differences that have a direct effect on the planning of our healthcare services. We are not just talking about the obvious need for separate maternity wards. The statistics of care form the basis of federal government policy-making. It takes years to train doctors and bring them up to a reasonable level of competence. Buying the land and getting zoning clearance for building hospitals and clinics is also a long-term task. Funding must be put in place, and pressure to get the projects through local and state procedures must be maintained. Think about it. There are more births than deaths every year. As people grow old, they use hospital services more often. Where are all the healthcare services going to come from over the next decades?

Perhaps surprisingly, more women than men use healthcare services to report disease or disorder. Ignoring accidents which tend to be men driving into each other, women take up more beds and are prescribed more painkilling medication than men. When it comes to pain, women seem more sensitive. The US box office is currently under assault from Saw 3D - presumably the last in the series. In this, there's a big irony because Saw is getting all the headlines while, lurking away in the art houses, you also have 127 Hours which is a true story. You can watch a mountain climber who, when he got trapped in an isolated canyon, cut off his arm to escape.

We tell you this, not to promote self-treatment, but to introduce the idea of acupuncture. Here specialists take a set of long needles and then drive them into your skin. Unfortunately, those of you who like horror movies will be disappointed at the outcome. Instead of rivers of blood erupting from gaping wounds like the oil from a gusher, nothing appears to happen. The victims lie calmly watching the needles go in, all the while chatting about the latest fashions or the current World Series with the expert. It's all very disappointing except, if you talk to any of the millions of people in China or South East Asia where acupuncture is a routine part of medicine, they all report excellent results from the treatment.

Indeed, in China, acupuncture is used alongside conventional anesthetic. Patients going through surgery remain conscious and discuss the finer points of the surgeon's technique with the theater nurses. To those of us in the West, this is taking things too far. The very idea of it really does seem like the plot from a horror movie. So we are not asking you to believe in anything so extreme. Rather we come down a few notches in ambition and report the latest results of a clinical trial in Seattle. A team of dedicated scientists gathered a group of 650 people, all of whom had serious lower back pain with loss of mobility. They were divided into four groups. One received genuine acupuncture from an expert. The second was pricked by toothpicks in all the right places. The third was pricked with needles at random (faintly disconcerting). While the final group was given conventional physical therapy.

In follow-up interviews over the next twelve months, the participants were asked about the level of pain and whether there were any improvements in mobility. Less than half those receiving physical therapy reported any significant change in their condition. But 60% of those receiving the "acupuncture" treatments reported significant and lasting improvements. So the results show a very strong placebo effect for both real and simulated acupuncture. The participants were prepared to believe they were going to find relief, and that's what they got. This strongly suggests the mind is a key player when it comes to lower back pain and, if people believe in the treatment, they get up to twelve months of benefit. New evidence supporting acupuncture

For centuries, Traditional Chinese Medicine (TCM) has been using acupuncture to treat a wide range of different diseases and disorders. Put most simply, millions of people accept treatment based on sticking needles into their bodies. This sounds a strange thing to do. Western doctors have been consistently skeptical yet, when you actually look at the theory behind it, there is a remarkable similarity in approach. TCM relies on mapping the human body to identify the critical points for manipulation. These points are essentially the same as the major groupings of nerves recognized as significant in the West. Pain-blocking injections are made into TCM acupuncture points. With reluctance, Western researchers are looking more closely for evidence whether acupuncture is effective. Reluctance because the major drug manufacturers discourage research that might put them out of business. Recently, a clinical trial in Seattle was written up in the Archives of Internal Medicine. Some 650 people suffering back pain were divided into four groups. One received genuine acupuncture, the second a modified version of acupuncture pricking the skin in the right places with wooden toothpicks. The third group were pricked with needles at random. The fourth received conventional physical therapy.

All the participants were given ten sessions over a seven-week period. There were follow-up interview spread over the next year to establish whether there was any lasting effect from any of the treatments. Interestingly, 60% of those receiving the different versions of acupuncture were still reporting benefits at the end of the year. Only 40% of those receiving physical therapy reported any continuing improvement. At one level, this would suggest there is a strong placebo effect from the use of acupuncture, i.e. those who receive the treatment believe it will be effective and so it proves to be. In scientific terms, there's nothing wrong with this outcome. When 60% of participants report a better quality of life twelve months after the treatment ended, this is strong evidence of the effectiveness of the treatment. Physical therapy is also based on social contact with one human selling the effectiveness to another yet this got a smaller placebo effect.

It's notoriously difficult to diagnose the cause of back pain. This suggests doctors should be very open-minded when looking for possible treatments. When the mind and body are so closely involved, engaging the mind with acupuncture, no matter whether real or simulated, seems more effective than conventional Western therapy. This is not to deny the role of a painkiller like Tramadol. When the pain grows more serious and mobility is affected, everyone will benefit from taking the drug. But this resort to a drug should be seen as part of a general strategy to encourage mobility. If this is your back, you can take the Tramadol for a short period of time. This should get you through the program of physical exercise, stretching, and hot and cold therapies designed to reduce any swelling and develop better muscle tone to support painless movement. If your problem of back pain is persistent, you should talk to your doctor about acupuncture and some of the alternative therapies. You may find more relief from these treatments than you expect.

Oh, cuddly furry squirrels!

One of the really great things about cartoons is how adorable they make smaller animals. Chipmunks, mice and canaries are forever doomed to be cute. Even Pep? Le Pew did wonders for the rep of skunks (at least the French branch of the family). It's hard to say why this should be. Most of them are pests and animators really should know better. But, given a choice of the animal kingdom, small children are expected to keel over in a sentimental mush at the idea of picking one of these creatures up and giving it a hug. They are given a starring role and Toys R Us gets filled up with the stuffed furry character from the latest movie (that or you get a plastic replica with your burger).

Perhaps the animators should leave their air-conditioned offices and walk out into the Californian countryside for the inspiration for their next blockbuster. Did you see the news out of the Angeles National Forest over the last weekend? On a regular basis, the Rangers trap squirrels and other rodents. They take samples and send them off for testing. About two weeks ago these dedicated public servants caught a squirrel wandering around the Los Alamos Campsite. The results of the test just came back. This cuddly creature was carrying the plague. Needless to say, our Rangers have closed down the campsite and quarantined the area.

So let's get technical. You all remember those boring history classes in school. Europe and the Black Death, Bubonic Plague and lots of people dying in the Middle Ages. That seemed all so yesterday, something to doze through and remember for the sake of passing the test. But the plague is very real and it's alive and well and living in California. That said, the closure of the forest site is preventative. Keeping people away, reduces the risk. The plague is spread when we are bitten by the fleas carried by rodents and cuddly small mammals. So if one of your children touches a squirrel or lies on the grass near a tree or burrow, fleas can jump on to feed. One of the problems is that the symptoms don't show for between 2 and 10 days, so you could be miles away before you realize. Then it's a quick dose of Doxycycline. For maximum effect, you should take this antibiotic within the first 24 hours of the symptoms appearing. If there's a delay, the death rate exceeds 50%. Reassuring, huh!

Keeping this article real, actual cases of the plague in the US are rare. It's found in Arizona, California, Nevada, New Mexico, and Utah. This is one of those things it's easy to say, but harder to do in the real world. It's like the equally annoying, "Pull yourself together". As if you had not realized you were a mess and needed to do something about it. Well, if you were able to do any pulling in the right direction, you would have done it weeks/months ago. So how do we recognise SAD? Well, at one time or another, we all get worried about appearing in public. Some shake at the thought of having to speak in front of an audience. Others worry if they have a job or some other duties involving meeting and interacting with people. "What will these people think of me? Will they find fault with what I say or do?" But it becomes SAD when you get way beyond simple shyness or nervousness. This is the type of anxiety that starts weeks or months in advance of any "event". It's usually one or two specific types of activity. Most social "everyday" living will be unaffected. We don't stop going out shopping because we fear the mailman knocking at the door. But if you know you have to make a presentation to a meeting or take responsibility for a function or occasion, then the anxiety can build to traumatic levels. This happens even though the occasion should be joyous, e.g. the rest of the family coming round for Thanksgiving.

No one knows for certain why this happens but, when it does, the approach is a combination of drugs and counseling. The doctors try to deal with this through "talk" therapy first. Research into lower back pain

If you ask any doctor for the standard treatment for the pain, stiffness and occasional spasms associated with lower back injuries, the automatic answer will always be the use of a muscle relaxant. But it was unclear whether the results were achieved by some quality in these drugs or merely because they have a sedative effect and the enforced rest speeds healing. To test out the reason for the effectiveness, a group of injured Mexican workers, all of whom had an acute lumbar strain, were divided into groups and given either a muscle relaxant, a sedative or a placebo. The dosage of the sedative was adjusted so that the degree of drowsiness would match that of the muscle relaxant. The participants were evaluated after two and four days, looking for a subjective assessment of the level of pain, an objective measure of the limitation to movement by using the finger to floor test, and whether there was any anxiety. The results showed a significant improvement from the group taking the muscle relaxant, particularly when it came to the objective measure of mobility. In the test of forward flexibility requiring the participants to reach down to the floor, they showed the greatest improvement. At the end of four days, the muscle relaxant outperformed the other two comparators by the proverbial country mile. Even though, by accident, the group taking the muscle relaxant also had the more severe initial symptoms, there was clear superiority. This confirms muscle relaxants as an active medication. The benefit is not achieved passively by sedating the patients, but rather by relaxing the muscles and so relieving pain and reducing the risk of spasms. Based on this evidence, muscle relaxants are the standard first response to all acute musculoskeletal injuries. Hence, whenever you are injured but expect to make a relatively quick recovery, you will be given a drug like Carisoprodol almost immediately after the injury and during the short period you will be instructed to rest. The idea is to encourage all the musculature in and around the affected area to relax and begin the healing process. Poison Oak

Poison Sumac

This is a proper tree, growing up to 20 feet tall, that has leaves made up of 7-13 leaflets. It is almost always an odd number of leaflets, unless there is a mutation. It has the same allergic properties as poison ivy, because it makes the same oil: urushiol. There is one major difference though, and that is that poison sumac can be deadly.

If an unfortunate hiker or camper inhales smoke from using poison sumac as fire wood, he or she will suffer extreme pain and severe respiratory difficulty that, if untreated, can result in death.

The Cure

Allergic reactions are basically a confused response from your immune system, so Prednisone, an immunosuppresant, is used to return the immune system to normal function.

It works like this: you take Prednisone, it gets to the liver where it is converted into prednisolone, an active steroid, which then tells the adrenal gland to chill out, and then all is better!
